Healthcare professionals who provide you with care are required by law to maintain records about your health and treatment or care you have received within any healthcare organisation. These records help to provide you with the best possible healthcare.

Home Vaccinations UK collects and holds data for the purpose of providing healthcare services to service users. We may collect information about service users which helps us provide patient centred care, respond to queries or send correspondence to other healthcare providers with the consent of the service user (GP surgery). We may keep confidential information in written form and/or in digital form. The records may include basic details about the service user, such as the name and address. They may also contain more sensitive information about the service users’ health assessments carried out in order to safely carry out the clinic in question.

2. Information we collect about you:

Home Vaccinations UK provides a Private healthcare.

Whilst you are registered, we will keep records of any healthcare and treatment you have received from Home Vaccinations UK. These records will help to provide you with the best possible healthcare.

3. Records we may hold about you include:

  • Personal information  - name, date of birth, gender, address and contact details such as email address and home and telephone number(s).

  • Information about medical or health conditions including allergies.

  • Information regarding your travel plans and your itinerary whist you are traveling.

  • Information about your next of kin or other emergency contacts.

  • Information received in response to any surveys, complaints claims.

  • Information about your consultation including dates, time, location, prescribed medication, Healthcare professional who carried out your consultation or follow-up appointment.

 

4. How we keep your information confidential and safe:

ALL working for an establishment participating in patient service is subject to the Common Law Duty of Confidence. Information provided in confidence will only be used for the purposes advised with consent given by the patient, unless there are other circumstances covered by the law. Home Vaccinations UK complies with the Care Quality Commissioners Regulation 17: Good Governance. All staff are required to protect your information, inform you of how your information will be used and only you can decide if and how your information can be shared. We hold information in accordance with the Data Protection Act 2018 and the General Data Protection Act. We will only ever use or pass on information about you if others involved in your care have a genuine need for it. We will not disclose your information to any third party without your permission unless there are exceptional circumstances or where the law requires information to be passed on.

5. How we use your information:

We primarily use information to enable our clinicians to provide an individualised service tailored to your needs. However, we also use your information to improve our services by:

  • Reviewing the care you received through clinical audit

  • Investigating queries, complaints and legal claims

  • Ensuring we receive payment for the care you receive

  • Preparing statistics on performance

  • To support the provision of your consultation and your care. We may often use third party providers to obtain medication. Third party providers will have access to your consultation form. They will review your form and prescribe the medication requested if it is safe to do. The pharmacy will then dispense and deliver the medication to Home Vaccinations ltd. 

  • To decide how best protect you and to provide the best treatment to you.

  • To make decisions at your request during the course of your treatment.

  • To keep your records up to date.

  • As necessary for our own legitimate commercial and compliance interests or those of other persons and organisations associated with us.

  • For compliance, accounting, managing and auditing our clinical and business operations.

  • To monitor emails, calls, other communications, and activities on our networks and systems.

  • For market research, analysis and developing statistics for improving our business and clinical performance.

  • To comply with our legal and regulatory obligations.

  • To allow us to investigate, address, respond to and resolve any complaints, legal claims and data protection breeches or clinical incidents.

  • It is important to note that your information will be stored on a Google Cloud system and kept for the duration that is required. Once this duration has been completed the documentation will be removed. 

  • Data transfer are restricted to EU and UK countries as outlined in the Data Protection Agreement on Google Workspace. 

 

 

6. Storing your information:

7. Sharing your information:

​

We may share your personal information with other NHS organisations, e.g. General Practice, Hospitals and Ambulance Services where they are directly involved in your care. We may need to share information from your health records with other non-NHS organisations, including Social Services. However, we will not disclose any health information to third parties without your explicit consent, unless there are exceptional circumstances, such as when the health and safety of others is at risk or where the law requires us to do so.

We may also be asked by statutory bodies to share basic information about you, such as your name and address, but not sensitive information from your health records. This is usually required to assist them to carry out their statutory duties. In these circumstances, where it is not practical to obtain your explicit consent, we are informing you through this notice, which is referred to as a Fair Processing Notice, under the Data Protection Act that we may share that data.

These non-NHS organisations may include, but are not restricted to:

  • Social Services

  • Education Services

  • Local Authorities

  • Police

Subject to applicable data protection laws we may share your personal data with:

  • Other healthcare providers with your consent, e.g. your GP

  • Administrative, support and reception staff within the Home Vaccinations UK Limited. These employees will have access to some patient data to help us perform our services. E.g, helping to book or reschedule your appointments, helping to prepare your invoices.

  • Our legal and other professional advisors appointed by us, for example our auditors.

  • Fraud prevention agencies, credit reference agencies, and debt collection agencies.

  • Government bodies and agencies in the UK where appropriate.

  • The Court Service, to comply with legal requirements, and for the administration of justice.

  • In an emergency or to otherwise protect your vital interests.

  • To protect the security or integrity of our business operations.

  • Payment systems and providers.

  • Any other party where we have your consent or as required to disclose such information by law.

Please be aware that if you choose to opt-out of sharing your information, NHS healthcare staff caring for you outside of this clinic may not be aware of the treatment you have received, any allergies or reactions as a result of the medication received through this service. If you wish to opt-out of sharing your information then inform us.

We may also share your information with third party providers we are affiliated with in order to obtain the stock you have agreed to have administered to you by Home Vaccinations ltd. These third party providers will have access to your consultation form, who will then prescribe and deliver the necessary stock if clinical appropriate.

8. Safeguarding

​

To ensure that adult and children’s safeguarding matters are managed appropriately, access to identifiable information will be shared in some limited circumstances where it is legally required for the safety of the individuals concerned.

9. Other organisations

​

The clinic may be asked to share your details with other organisations in the form of private medical reports or extracts from your medical records – perhaps to assist you with a private insurance payment. We will only do this once we are in possession of your explicit consent and you are entitled to a copy of your records we pass on.

10. Access to your information

​

Under the General Data Protection Regulation everybody has the right to see or have a copy of data we hold that can identify you, with some exceptions. You do not need to have a valid reason to access your information. If you wish to access your data, you must make the request in writing. 11. Mobile telephone number

If you provide us with your mobile phone number we may use this to send you reminders about your appointments, links for reviewing the service or other health screening information. Please let us know if you do not wish to receive reminders on your mobile.

 

12. Notification

The General Data Protection Regulation requires organisations to register a notification with the Information Commissioner to describe the purposes for which they process personal and sensitive information. We are registered as a data controller and our registration can be viewed online in the public register at:
